Parents should submit all forms to the Admissions Office before the student begins the first day of class:

• Application Form
• Early Childhood Questionnaire (EYP, PK, K)
• Early Childhood Questionnaire (maternelle) french version
• Teacher Recommendation form Grades 1-5
• Teacher Recommendation form Grades 6-8
