CHECKLIST

Enrolment will not be considered until all of the following information has been received.

• Completed application form
• Non refundable application fee. See current information on fees
• Passport size photo of applicant
• Copies of three years' school records. Applicants younger than Grade 3 will submit all previous school records. All records must be translated into English
• Copies of any standardized testing
• Early Childhood Questionnaire (for applicants to EYP, PK, K)
• Teacher letter(s) of recommendation
• Hand-written writing sample (one-page autobiography for students grades 5-8)
• Applicants to Learning Support Services (Resource and ILC should submit copies of their IEP (Individualized Educational Plan) and copies of all previous psycho-educational reports / assessments.
